Would you Like to Assist Individuals with Disabilities Obtain Timely Medical Care?
Then the Surrogate Decision-Making Committee (SDMC) may be the right fit for you! We are working on recruiting caring and knowledgeable volunteers in the medical, legal, and advocacy field for our program.
Our program is an alternative to the court system. It provides consent for non-emergency major medical treatment and end-of-life care decisions for individuals with disabilities who lack the capacity to provide consent and do not have a surrogate decision-maker to act on their behalf.
Who can Serve as an SDMC Volunteer?
SDMC relies on Volunteers who are New York State residents from the following categories:
- NYS-licensed health care professionals
- NYS-licensed attorneys
- Former patients or family members of individuals with disabilities
- Advocates for persons with disabilities
How to Become a Volunteer?
Prospective volunteers must submit an online application and complete the required Online Volunteer Training. The online training is approximately one hour and thirty minutes in length.
